NEW CLIENTS

Getting started with services is quick and easy. In order to verify insurance benefits please provide a photo of the front and back of your insurance card, your name, address, phone number, and DOB (if you are not the primary on the insurance policy, please provide the primary's DOB). Please provide the above information via Text (224) 308-4108 or email shamariservices@yahoo.com.   Once we receive the required details, we will verify your insurance benefits and reach out to schedule your first session.

If you are not using insurance to pay for visits, please call, text, or email requesting to schedule a New Patient Appointment.

We are proud to be paperless. Once we schedule your first session, we will send you a link to digitally submit our new client forms and consents.
